You can download the BASI membership form through this web site. You will need software capable of displaying an Adobe PDF file (standard on newer computers) and a printer.

This is a free download. Follow the directions at the Adobe web site to install the reader.

Navigate within the form by using the mouse or TAB key. You can check and uncheck the Yes / No / Selection boxes with the mouse or space bar on your keyboard.

After you complete the membership form print two copies. One is for your records, the other for BASI. Sign the BASI copy and send it to the BASI office with your payment. Please do not mail cash.
